If you need Certified Copies of Vital Records, download the following form:

•  Certified Copies  (Birth, Marriage, Death) 

• The application may be delivered in person, deposited in the Municipal Building drop box located next to the front door, or mailed to:

  Whitehouse Station, NJ  08889

• Include the completed form, required documents to establish identity, and check or cash. The fee is $10.00 for each certified copy. There is a $5.00 additional charge to apply by mail.
• For Certified Copies of a Marriage Certificate the fee is $10 per copy
• For Certified Copies of a Death Certificate the fee is $10 per copy
• For Certified Copies of a Birth Certificate the fee is $10 per copy
